Chengdu Tianfu New District social security claims need those materials

Social security medical insurance reimbursement materials to be provided:

Transfer certificate issued by the hospital in the city;

Take the transfer certificate issued by the hospital to the city and district social security office (medical insurance office) for approval and filing of medical treatment in a different place;

Original hospitalization invoice of the designated hospital in a different place;

Original machine-printed list of expenses;

Valid copy of hospitalization medical record (valid hospital seal) 1 copy;

1 copy of ID card.

Reimbursement procedures for overseas visits:

Bring the patient's ID card, two one-inch color photos, and the NPPC medical card to the county co-management office for the referral filing procedures;

Bring the patient's ID card, the NPPC medical card, and the referral filing procedures to the referral hospital and go through the NPPC hospitalization procedures;

After the discharge from the hospital, the patient will be discharged from the hospital, with the patient's own ID card (or hukou), the NPPC medical card, medical records, and the hospitalization procedures.

After being discharged from the hospital, the patient will be reimbursed by the Cooperation Administration Office with his/her ID card (or hukou book), New Rural Cooperative Medical Card, a copy of his/her medical record, hospitalization statement (in the form of an invoice, if any), list of hospitalization expenses, and referral filing procedures.

Remarks:

Participating migrant workers who seek medical treatment at their workplaces may first seek medical treatment, and then go to the county cooperative management office to make up for the referral filing procedures during the period of hospitalization or after being discharged from the hospital;

The unit of medical treatment must be the local New Farmers' Cooperative designated medical institution; otherwise, no reimbursement will be made.
