What should I do if I lose my medical insurance card in Jinan?
After Jinan medical insurance card is lost, it needs to be reported immediately. There are three ways to report the loss, as follows:
Telephone loss reporting:
1. If the medical insurance passbook is lost, you can call 6 168 177 to report the loss;
2. When reporting the loss by telephone, you must provide the name, gender, citizenship number and unit name of the person who reported the loss;
3. Stop the settlement function of the card through the medical insurance network within 24 hours after the telephone report loss is confirmed;
4. After the loss is reported by telephone, the person who reported the loss shall go through the formalities of reporting the loss in writing within two working days. If the loss is not reported in writing within the specified time, the municipal medical insurance agency shall restore the settlement function of the original social security card.
Note: The economic losses caused by the card shall be borne by the individual before accepting the telephone report of loss and the settlement of card suspension. The economic losses caused by the failure to report the loss in writing in time and the automatic decoupling of the system are also borne by the individual.
Written loss report:
1. After losing the medical insurance passbook, you should report the loss in writing to the medical insurance agency with your ID card in time;
2. When reporting the loss in writing, the insured person shall fill in the report on reporting the loss of the social security card for urban workers;
3. After accepting the written report of loss, immediately stop the financial settlement function of the card through the financial network of China Bank, immediately freeze the personal account of medical insurance through the social insurance network, and stop the settlement function of the card within 24 hours.
4. Note: (1) The insured can go through the written procedures for reporting the loss at any medical insurance agency and BOC outlet in the urban area. (2) If entrusting others, the client shall show his identity card and register.
(3) The person who reported the loss shall go through the formalities of card replacement in time according to relevant regulations.
